CARES (Concern Awareness REsponse Support) Referral Form

The purpose of the KSC CARES program is to provide a means for early intervention of at-risk students through collaboration with campus departments, faculty, and staff. The KSC CARES Team is a multi-disciplinary body of stakeholders from across the College who receive referrals pertaining to students of concern, collects additional information and identifies and enacts appropriate strategies for addressing the situation. Students exhibiting behaviors that are of concern in relation to their academic, personal, physical, and emotional well-being should be referred to this team of professionals. The CARES Team is not meant to be the sole mechanism of communication and will not take the place of services provided by the Counseling Center, Student Conduct, Campus Safety, or other established student services. Anyone (faculty, staff, or student) can submit a CARES referral form.

Mental Health and Well-Being (Counseling Center): As a Keene State College student, we believe that your personal health and well-being is equal in importance to your academic success. Because life just comes with challenges, there may be times when you experience struggles with your emotional health that interfere with academic experiences and/or negatively affect your daily life. If you or someone you know needs support, the Counseling Center at KSC offers professional, free, confidential assistance. They are located on the 3rd Floor of the Elliot Center or can be contacted at (603) 358-2437. Also, a crisis counselor is available after business hours and on weekends at (603) 358-2436. Remember, it is okay to need help and it’s a strength to reach out for it.

Financial Aid - helps students in financial need by providing workable packages of merit scholarships and grants, loans, and campus jobs. (Not to be confused with Student Accounts)

Global Education Office (GEO) – includes both international and national (US) study away.

The Hungry Owl: If you are facing challenges securing food, you are strongly encouraged to make use of the free food provided by The Hungry Owl food resource at a variety of locations on campus including the main location behind Randall Hall and at Mason Library. For housing and other issues that you believe are affecting your academic performance, contact the Dean of Students for support (deanofstudents@keene.edu or 603-358-2842). Office of Disability Services: If you are a student with a diagnosed disability – You are responsible for making an appointment the Office of Disability Services (ODS), located on the first floor of the Elliot Center (603-358-2353), to discuss eligibility requirements, including a review of necessary supporting documentation, and appropriate academic accommodations that you may require. Requests for academic accommodations should be made during the first two weeks of the semester, except for unusual circumstances, so arrangements can be made. It is entirely your responsibility to contact the ODS and concurrently supply your faculty with Faculty Notification Accommodation letters which will inform them of exactly the accommodations to which you are entitled.
