1. If you are diagnosed with a disability, please make an appointment to meet with the Office of Disability Services (ODS), first floor in the Elliot Center, ext. 8-2353. All students with the necessary supporting documentation will be provided appropriate accommodations as determined by ODS. It is entirely your responsibility to contact the ODS and concurrently supply me with your Faculty Notification Accommodation letter, which will inform me of exactly what accommodations you are entitled to. You will only receive accommodations once you provide me with the Faculty Notification letter.

2. It is Keene State College policy to provide reasonable accommodations to students with disabilities. If you would like to request such accommodations, please contact the Office of Disability Services (ODS) located on the first floor of the Elliot Center or call 358-2353.

3. If you are a student with a disability: The Office of Disability Services (ODS), first floor, Elliot Center #358-2353, is available to discuss eligibility requirements and appropriate academic accommodations that you may require as a student with a disability. Requests for academic accommodations should be made during the first two weeks of the semester, except for unusual circumstances, so arrangements can be made. You are responsible for making an appointment with ODS for disability verification and determination of reasonable academic accommodations.
