Care Manager (Child & Family)
Responsibilities include coordinating and assuring the continuity of services for each client on their caseload. Work with each client to coordinate care planning, complete a comprehensive assessment of needs, develop a care plan and monitor the effectiveness of it, and ensure resource linkage, referral and advocacy. Coordinates care and maintains client's medical records within the agency.
Valid driver's license, proof of adequate automobile liability insurance and reliable transportation is required.Valid driver's license and vehicle required. Candidates are subject to a BEAS, Criminal and Motor Vehicle check.
BS/BA in social work, rehabilitation, psychology, education, or other related human services field accepted. Must be able to exercise independent judgment and use discretion in the performance of these duties while operating within the limits of standard practice.
