Worker's Compensation

Any employee (benefits-eligible, temporary, non-benefits-eligible and/or student), who is injured during a working period, is required to seek immediate medical attention and promptly report the injury to his/her immediate supervisor.

Reporting a Work-related Accident

In the case of an accident involving a Keene State employee (staff, faculty, ancillary, student workers, etc):

1. Notify Campus Safety. They will help get emergency assistance to the scene and provide support as necessary.

2. Employee and supervisor complete the Report of Injury/Occupational Illness Form and return to Human Resources within 2 working days of incident. Please let the doctor know it is a work related injury and include any medical documentation with your report. Employee should contact Karyn Kaminski 603-358-2486.

3. Supervisor will complete the Accident, Near Miss or Potential Hazard Inquiry Form within 2 working days of the incident and forward one copy to Human Resources MS-1604 and one copy to the Health and Safety Committee MS-2502. Note: Because of the short turn around time, you may want to identify who will prepare the investigation if you are absent. Such delegation will not relinquish you of responsibility for the investigation or reporting.

All emergency medical needs should be sought at the nearest hospital emergency room or clinic. All injured employees will receive information from their direct supervisor regarding the Managed Care Program directed by our Worker's Compensation insurance carrier, MEMIC Indemnity Company. All injured employees must seek non-emergency medical treatment through a provider which has been approved by MEMIC Indemnity Company or Windham Group (WIN). An Injury Management Facilitator can be reached at 866-636-4292 or 800-898-0386 if there are any questions. Names of approved providers can be obtained through the UNH Office of Environmental Health and Safety web site.

All questions or concerns regarding the reporting or treatment of occupational injuries or illnesses should be directed to Karyn Kaminski 603-358-2486.
