New Student Information

Please note that Health forms are due by July 15, 2008.

Our health form must be completed and returned to satisfy state public health requirements, Keene State College Pre-entrance Requirements, and the American College Health Association Pre-matriculation Immunization Recommendations. Please submit to Keene State College, Health Services, 229 Main St., Keene, NH 03435-2802.

If for any reason you need a copy of the health form, you can download it. If you have problems downloading the form you can request the form from Maria Sibley. Please include your phone number and complete mailing address.

Requesting Copies of Medical Records Medical Records are kept for a period of 7 years. If you need a copy of your records you must send a signed note authorizing KSC to release your records. Please specify the name, address and fax number of the individual or office the information should be sent to.

Summer Medical Care For medical care during the summer months, consult your physician or contact Cheshire Medical Center’s Emergency Care Center, 580 Court Street, Keene. Emergency service is available 24 hours a day, 7 days a week through the Emergency Room. For questions or information you may call 354-6600. Administrative services are available Tuesday, Wednesday and Thursday from 8:00 am to 2:30 pm.

Our health form, “Report of Health History” including Immunization Verification, must be completed and returned to satisfy state public health requirements, Keene State College Pre-entrance Requirements, and the American College Health Association Prematriculation Immunization Recommendations.

You should have received our health form in the mail. If for any reason you need a copy of the health form, you can download it here. If you have problems downloading the form you can request the form via email to msibley@keene.edu. Please include your phone number and complete mailing address.

Athletes

If you think that you will be participating in any sports activities it is important that you submit a copy of your health form to: Keene State College, Bob Merrow, Training Room, Spaulding Gymnasium, 229 Main St Keene, NH 03435-2301.

Some students may have had physical examinations completed for other reasons prior to KSC acceptance. We will accept forms for physical examinations completed up to one year before coming to KSC. This information must include immunization verification.

Transfer Students

We will accept a physical exam and immunization record from your previous college or university if it has been completed within one year.

Required Immunizations for all Students

MMR (Measles, Mumps, Rubella) 2 doses required

Varicella (Chicken pox)

Meningococcal (Meningitis)

TB Test (required if high risk: see Tuberculosis Risk Questionnaire)

Strongly Recommended Vaccinations for all Students

DPT (Diphtheria/Pertussis/Tetanus) primary serie, including Tetanus booster within last 10 years or Tdap.

Polio

Hepatitis B Series (3 doses)

HPV Series (3 doses, if female)

Health Insurance

It is important that each student at Keene State College have health insurance for coverage of care not received in the Center for Health & Wellness. We encourage that each student be made aware of their health insurance provider, coverage and limitations. It is of particular importance that students know about emergency care coverage, prescription coverage and limitiations. Please provide your college student with an insurance card and phone numbers.

Things to Bring

Don’t forget your prescriptions, prescription information, items from the list below, and your insurance coverage provider information. Be sure you know who your medical insurance provider is, what numbers, forms, and procedures need to be followed to submit claims for medical expenses incurred outside of KSC Center for Health & Wellness.

Don’t forget to bring...

  • non-aspirin pain reliever: acetaminophen and/or ibuprofen (ex: Tylenol, Advil, etc.)
  • decongestant (ex: Sudafed, etc.)
  • cough medication with expectorant and cough suppressant (ex: Robitussin, Formula 44, etc.)
  • antacid liquid (ex: Peptobismol, Mylanta, etc.)
  • throat lozenges (ex: Vicks, Ricola, etc.)
  • antibiotic ointment (ex: Bacitracin, Neosporin, etc.)
  • bandaids
  • athlete’s foot medication (ex: Tinactin, Desenex, etc.)
  • nail clippers
  • heating pad
  • cold mist vaporizer
  • thermometer

Alcohol Wise Assignment

Enrollment Instructions

To participate in the Alcohol Knowledge Challenge, you will need access to acomputer connected to the internet and an e-mail address to complete Alcohol-Wise, the online alcohol prevention course.

  1. Go to 3rd millennium classrooms
  2. Click on Begin Enrollment on the left side menu. Complete enrollment information under New Student. Remember to use your ksc.mailcruiser.com email account to receive credit.
  3. When you are directed to the Enter Control Number page, enter the code: KSCAW

Do not include payment or credit card information on the enrollment form.

You will receive a password immediately on the screen and by email.

To begin the course: Login using your ksc.mailcuriser.com email address and the password given to you by 3rd Millennium Classrooms. All individual responses are confidential and not associated with your name.

The course takes about one hour to complete and you can login and out as needed. Although if you log out during a section and have not completed that section you will have to restart that section.

You will receive an email in 60-days to complete a 15 minute follow-up.

Print out a copy of your completion for your records.

Don’t delay and complete the program by July 1st to be entered to win an I-POD Nano or cost equivalent in I-tunes music! 2 lucky winners will be drawn!!!

e-CHUG / e-TOKE

What is the e-TOKE?

The electronic THC Online Knowledge Experience (e-TOKE) is a marijuana-specific brief assessment and feedback tool designed to reduce marijuana use among college students. The on-line assessment takes about 10-15 minutes to complete.

What is the e-CHUG?

The e-CHUG (electronic Check-Up to Go) is an interactive web survey that allows college students to enter information about their drinking patterns and receive feedback about their use of alcohol. The on-line assessment takes about 6-7 minutes to complete, is confidential, and personalized to the individual.

What do they do?

When participants access the e-TOKE, they are prompted to enter information about themselves. In addition to demographic information, they enter information about their use of marijuana, spending patterns, and their lifestyle. After all information has been entered, the form is submitted.

The information you entered is validated and then processed. The e-TOKE calculates a number of variables and compares individual responses to national and local college norms. Then, that information is displayed in standard HTML format.
