Students and Parent PLUS loan borrowers are encouraged to complete and submit the forms below to our office. However, submission of these forms is optional.

Student Credit Balance Authorzation (pdf)

• For students with federal financial aid, we are required to notify you about this form.
• Federal funds may only be used for specific account charges including tuition, mandatory fees, room, and board.
• By completing this form, you are stating that you approve of using your federal financial aid credit balance to pay all other related institutional expenses such as library fines, ID replacements, health service fees, etc., that do not exceed $200.
• You may receive a refund check and have a balance due on your account if you have not completed this form.

PLUS Loan Credit Balance Authorzation (pdf)

• For Parent PLUS loan borrowers, we are required to notify you about this form.

• By completing this form, you are stating that you approve of using your Parent PLUS loan credit balance to pay all other related institutional expenses such as library fines, ID replacements, health service fees, etc., that do not exceed $200.

Refund Request Form (pdf – download to fill in)

Students may want to submit a Refund Request if they want their refund check:

• Made payable to someone else, or
• Mailed to an alternate address (other than the home address).
