Workstation Installation Scheduling Form

Prepare for your Upgrade

  • We will transfer any items on your Desktop, your Outlook profile (mailboxes, calendar, personal folders, contacts), your Firefox bookmarks and Internet Explorer favorites, and other standard application data. If you are a former Netscape user, we will import your Netscape bookmarks into the browser of your choice (Internet Explorer or Firefox).
  • If you have any peripheral devices such as a local printer, PDA, scanner or special mouse, please locate the software for those devices before your installation date.
  • NOTE: If you have any personal files that are not work related (like iTunes or pictures) on your computer, please back them up before your scheduled appointment. The HelpDesk is not responsible for backing up or transferring personal files from your old workstation to the new workstation.
  • NOTE: You will be responsible for backing up any files related to non-standard software and the reinstallation of all non-standard software.

I will bring my laptop/desktop to the HelpDesk for my upgrade. I understand my password will be changed and I will be given the new password when my new laptop is delivered.

I would like the HelpDesk to pick up my old workstation and then deliver my new one. I understand my password will be changed and a voicemail will be left with my new password. I will leave any software required to install peripheral devices (local printer, PDA, etc.) in my office.

I would like the technician assigned to upgrade my computer to complete the upgrade in my office. I will be present during the upgrade so that I can logon to the new workstation as needed. I understand the entire upgrade process may take over an hour, depending on the amount of data that needs to be transferred.

List any non-standard software (i.e. software not on the standard software list). This is a list of the standard software that is installed on your new workstation. You are responsible for the installation and configuration of any non-standard software.
