Computer Loaner Request Form
IT Support Services has a small inventory of computers available on a temporary basis for faculty and staff to be used for emergencies or special requests, including but not limited to:
- a special event like registration, add/drop, and submitting housing applications
- a position is filled prior to a workstation being purchased for the position
- a faculty member on sabbatical takes his/her laptop and the replacement has no workstation
These loans are short-term, no longer than 1 academic semester, and you will be contacted to arrange the return of the workstation after the end date listed on this form. This is only a request form. Approval is dependent on the availability of a workstation.
