RN Case Manager
Summary: The RN Case Manager is an essential member of the interdisciplinary care team and is responsible for coordinating the interdisciplinary assessment and plan of care for hospital patients using a patient-centered, systematic approach consistent with the Standards of Practice adopted by the American Case Management Association (ACMA) within the domains of: Collaboration; Communication; Facilitation; Coordination; Advocacy; Resource Management; Accountability; and Professionalism.
R.N. licensed in N.H.; preferably with a B.S. in Nursing or Healthcare Administration.
A minimum of two years in direct patient care in an acute care setting is required.
Community based (such as physician practice, skilled care or home care) experience is desirable.
Experience in Care Coordination (required) and Utilization Management experience (highly desirable).
Case Management certification preferred.
